What is TRT?
Testosterone Replacement Therapy is a personalized treatment approach that addresses the symptoms of low testosterone or “low T”. Testosterone Replacement Therapy Options:

General Urological Issues Treated by Dr. Wierschem
BPH – Benign Prostatic Hyperplasia
Diagnosis/Evaluation may include: Detailed history, physical and prostate exam, PSA, urinalysis and possible other tests of bladder/urinary function.
Treatments for BPH may include: diet modification, medication, office based procedures and surgical procedures.
Prostate Cancer
Diagnosis/Evaluation may include: Detailed history including Family history, Physical exam to include the prostate, PSA testing, MRI evaluation and prostate biopsy.
Treatment for Prostate cancer may include: Active Surveillance/Watchful Waiting, Focal laser treatment, Surgical Removal, Radiation/Proton Therapy and medication.
Prostatitis
Diagnosis/Evaluation may include: Detailed history, physical and prostate exam, PSA, urinalysis and possible other tests of bladder/urinary function.
Treatment of Prostatitis may include: Antibiotics, Anti-inflammatories, prostate medication, diet modification and other behavior modification techniques discussed with the doctor.
Vasectomy
This procedure is performed in the office by Dr. Wierschem. It is a painless procedure that typically takes less than 10 minutes. You will be set up to have your semen checked 6-8 weeks after the procedure to ensure that you are sterile. Until that time you will need to continue contraception. Nephrolithiasis – Kidney stones
Diagnosis/Evaluation may include: Detailed history, Physical Exam, Urinalysis, blood tests, Radiological evaluation (e.g. plain X-ray, CT scan, Ultrasound), comparison to prior radiological studies, labs and reports.
Treatment of Kidney Stones may include: Active Surveillance/Watchful Waiting, diet modification, medical management, pain management, surgery (e.g. Shock Wave Lithotripsy (ESWL), ureteroscopy (typically with laser stone ablation), stenting (an internal tube that bypasses the obstructing stone) or even external drainage of the kidney in certain cases.
Infertility
Diagnosis/Evaluation may include: Detailed history, Physical Exam, urinalysis, semen analysis (and possible other semen studies), radiological evaluation.
Treatment of Infertility may include: Active Surveillance/Watchful Waiting, medication, diet and behavior modification and surgical intervention.
Each bioidentical hormone pellet is designed to last approximately 3 to 6 months, depending on individual factors such as metabolism, activity level and hormone needs. Dr. Wierschem will monitor your hormone levels and symptoms to determine the optimal timing for pellet replacement.
Like any medical treatment, BHRT can have side effects, though they are generally minimal and manageable. You may experience acne, testicular atrophy, slight fluid retention or changes in mood or energy levels initially as your body adjusts to the new hormone levels. Communicate any side effects to your healthcare provider, who can adjust your treatment plan accordingly.
BHRT can indirectly support weight loss efforts by addressing hormone imbalances that may contribute to weight gain, such as low testosterone levels. By restoring hormone balance, BHRT can improve metabolism, increase muscle mass and reduce fat accumulation, aiding in overall weight management.
Bioidentical hormones are chemically identical to the hormones naturally produced by your body, which often leads to better compatibility and fewer side effects. In contrast, synthetic hormones may have a slightly different chemical structure, which can affect how they work in the body. They're also associated with more severe side effects, such as an increased risk of blood clots.
